Print the form and complete to register for the swim team. One family per form. Return the form to the YMCA front desk, to Chris Elliot, Registrar, or to a swim coach.

Transportation and discipline acknowledgment form --
Print this form, read it thoroughly, complete and sign, and return it with the Stingray Registration form.
